Class 3 Error

SYMPTOM:
If a run, which includes periodic boundaries, also has mesh adaption, any new nodes that the refiner creates at a periodic boundary may be unattached at the corresponding periodic boundary. This can lead to the CFX Solver computing a solution in which only the original nodes on the periodic boundaries recognize the periodicity of the problem, and thus the solution will be corrupted.

PROBLEM:
This is due to a bug in

WORKAROUND:
No workaround is currently available. However, if the mesh refinement only takes place in regions away from the periodic boundaries, then there will be no error in the solution.

FIXED IN:
ANSYS CFX Release 12.0.
